ABSTRACT

Migraine is a type of Headache characterized by moderate to severe throbbing pain, generally associated with nausea, vomiting and light sensitivity. It can be correlated to Ardhavbhedaka, which according to the Ayurvedic science is caused by simultaneous vitiation of all the three Doshas viz. Vata, Pitta and Kapha. Motion sickness is also a common disorder wherein patient suffers from symptoms like nausea, vomiting, increased salivation, dizziness, upon getting triggered by certain movements. In this case study, a female patient of middle age group (Madhyam Avastha) suffering from long standing migraine and motion sickness since past 15 years (on and off), hampering her day to day activities, was administered with the treatment protocol as per Ayurveda resulting in significant relief to her symptoms. The treatment, planned according to her pulse diagnosis and clinical examination, was based on herbal supplements, Panchakarma (detoxification) in the form of Virechana, home remedies, Marmaa therapy, dietary and lifestyle modifications. The treatment was focused on removing the toxins (Aam) from the body with the help of detox (Virechana). This also resulted in alleviation of the vitiated Vata and Pitta Doshas and restoration of the Jatharagni (digestive fire). The treatment modality has been effective to reduce the episodes significantly.

Beauty holds a very important place in expressing the personality now-a-days. Clear and healthy skin makes a good impression. Everyone especially the young generations are most conscious and careful about the beauty of the face. Due to hurry, worry, and curry type of lifestyle and some hormonal factors in the adolescent age many people suffer through Acne. It is commonly seen in young population. Mukhadooshika, according to Ayurveda, can be correlated to acne. It is a type of Kshudraroga. It involves vitiation of Kapha and Vata along with Rakta resulting into formation of Mukhadooshika. Common sites are Mukha, Urah and Prushtha. In the management of Mukhadooshika, both Shodhan and Shaman chikitsa can be performed as per the Avastha. Vaman, Virechana, Raktamokshana, Nasya, Lepan etc. Panchakarma upakrama along with Abhyantar aushadhi chikitsa (internal medications) prove very effective in reducing the Mukhadooshika and preventing the recurrence. In this case study, a female patient with diagnosis of Mukhadooshika was treated with Virechana karma, Mukhalepa and Aushadhi chikitsa that showed marked improvement.

ABSTRACT

Bronchial asthma is one of the most distressing chronic illness in all socio- economic status people, affecting all age groups. Changing in lifestyle, demographic factors, urbanization, environmental charges are the triggering factors of bronchial asthma. Tamaka swasa is characterized by paraoxysmal attacks of breathlessness, cough, coryza, due to chest tightness, rapid respiration, distress inability to expectorate and prolonged expiration. Methods: This is a single case study of a 34 year male patient, came with the complaints of difficulty in Breathing since 5 years. He also complaints of abdominal distension and decreased appetite. According to patient, he was apparently well before 5 years. Gradually he developed breathing difficulty because of his smoking habit. Patient also c/o abdominal distension and decreased appetite at that time. In the case study patient was given Sadhyo vaman first followed by Snehapana with Brahmi ghrta was given along with Internal medications. Result: Patient showed remarkable improvement in severity of symptoms and as well as time period between 2 successive episodes of dyspnea is increased. During this course we observed that the patient had partial relief of symptoms. Lesser the chronicity greater the relief and improvement in chronic condition was observed. Interpretations and Conclusion: The patient was given Sadhyovamana, followed by Snehapana with Brahmi ghrtam for Virechana. The contents of Brahmi ghrtham are Ghee, Brahmi, Shankapushpi, Trikatu, Shweta trivrt, Krishna Trivrt, Vidanga, Nipadruma, Saptala, Danti, followed by Virechana with Trivrt lehaya. Tamake tu virechana In this condition Virechana is advised as the best line of treatment, and in the above mentioned case after Virechana patient feels better and got much more relief from symptoms.

ABSTRACT

In the midst of movement and chaos of life, we forget to pay heed to our health and as a result of which susceptibility to various allergens is increasing day by day. Allergens can be dust, smoke, fumes, pollen and even food. A person can either be allergic to certain allergen right from his birth or can acquire hypersensitivity in later stage of his life. Food allergy is quite common in present era and is affecting the lives of many people. Although there are a number of manifestations of food allergy like nausea, vomiting, diarrhoea, headache etc, most commonly seen are skin manifestations like blisters, itching, redness, burning sensation, oozing, discolouration etc. Acharyas have also mentioned Virudhaahar as one of the most prominent cause of Kushtha. Virudhaahar is any food that causes sensitivity reactions in a person. In contemporary sciences, there are no such permanent cures for hypersensitivity induced skin diseases. But in Ayurveda, through Shodhana and Shamana, these can be treated effectively and occurrence can also be avoided to some extent. This case report is of 38 yrs old male who visited our OPD with itching, oozing, blisters with intense burning sensation in palms and redness and itching in groin areas. The disease was diagnosed here as Charmdala. The treatment protocol opted was Virechan and Shamana. The patient was assessed every 7 days for a month and showed significant relief. This case thus proves that Ayurveda has the potential for curing Virudhaahar janya Kushtha which is often unresponsive to the contemporary treatment protocol.

ABSTRACT

Alcoholic hepatitis is an inflammatory condition of the liver. It is caused by excessive alcohol consumption over an extended period of time. Genetics, other liver disorders, and nutrition may also contribute to alcoholic Liver Disease. In Ayurveda there are so many herbs and natural remedies available for treatment of liver diseases. Herein we present a case of married male of age 36yrswho was reported in Kayachikitsa OPD, All India Institute of Ayurveda New Delhi India with chief complaints of pain in abdomen with mild distension, yellowish discolouration of eyes, skin and dark yellow urine, loss of appetite, disturbed sleep, pedal oedema, weakness, anorexia. The diagnosis alcoholic liver disease was made on clinical ground supported with Ultrasonography and blood biochemistry reports. Ayurvedic treatment given was Nitya Virechan with Trivrit Avleha (regular purgative), Bilwadi Gutika Anjana (medicated collyrium) and Shamanoushadhi (palliative drugs). During the treatment the patient was totally abstaining from alcohol. Within 45 days of starting the therapy patient showed significant improvement which were assessed by measuring liver functions through specific clinical features and laboratory parameters. Hence presenting this case is an evidence to demonstrate the effectiveness of Ayurvedic treatment in ALD which can be proved an important guideline for treating Alcoholic Liver Disease with safe and effective Ayurveda line of management.

Bronchial asthma is a highly prevalent disease and in most of the etiopathogenesis and symptomatology bronchial asthma can be co-related to that of Tamaka Shvasa. As the disease is originated from Pittasthana, Virechana Karma is indicated in the treatment of Shvasa Roga .In Charak Kalpasthana there are many Virechana Yogas are mentioned with different Kashay Kalpana. So, the present study is carried out to study efficacy of Virechana with Trivrutta in combination with other Drayas mentioned in Kalpasthana in the form of Leha Kalpana specifically indicated in Shvasa Roga. Aim: To study efficacy of Virechana karma with Trivrutta Leha Yoga in Tamaka Shvasa. Materials and Methods: Total 15 patients fulfilling the diagnostic criteria of Shvasa Roga were selected from OPD & IPD of Panchakarma Department. The Sign and symptoms mainly breathlessness, cough, ronchi were given scores depending on their severity. The patients were also assessed for Lung Function Test to determine pulmonary impairment. For the purpose of Virechana they were given Deepan Pachana with Trikatu Churna for 3 days then they were subjected to Snehapana with warm cow’s Ghee. The patients were given Abhyanga and Bashpaswedana for 4 days followed by Virechana with Trivrutta Leha Yoga and Sansarjana Krama. Follow up was done in an interval of 15 days for three times and scores were noted down before and after treatment. The scores were analyzed by using Wilcoxon Signed Rank Test for subjective parameters and Studant paired ‘t’ test for objective parameters. Result: The trial drug has shown statistically highly significant reduction in breathlessness, cough, rhonchi and significant changes in lung function tests. Conclusion: The formulation has proved its best purgative effect on bronchial Asthma and thus relives the symptoms of disease immediately. It has reduced frequency of episodes of breathlessness and severity of symptoms.

ABSTRACT

Alopecia areata is characterized by circumscribed non-scarring loss of hair in single or multiple patches over the scalp or body hair. Histopathology shows perifollicular and intrafollicular inflammatory cells infiltrate composed mainly of lymphocytes, described as swarm of bees’ appearance. The characteristic lesion is an asymptomatic, smooth bald patch with normal skin within the patch. Fine regular pitting of nails and tachynochia is a common association. When the hair loss involves the entire scalp it is known as alopecia totalis. Khalitya in Ayurveda is a disease in which Pitta combining with Vata and getting into the pores of hairs (hair follicles) cause fall of hair, and then Kapha along with Rakta obstructs the pores of those hairs and makes it impossible for other hair to grow. As it does not destroy the hair follicles the potential for the re-growth is maintained for many years. However growth in alopecia totalis and universalis is limited. Material and Methods: The present clinical study is a case report on the efficacy of Shamana (pacification) medication along with Vamana (therapeutic emesis), Virechana (therapeutic purgation), Basti (therapeutic Enema) and Nasya (therapeutic nasal medication) and Raktamokshan (therapeutic bloodletting) in the management of a patient diagnosed as Alopecia totalis. Results: There was significant re-growth of the hair of the scalp. SALT Score improved from 97.76 to 2. Discussion: The collected data from this study suggests that Ayurvedic Shodhana (Purification) treatment along with Shamana medications can provide an efficient result for managing Alopecia totalis.

ABSTRACT

Panchakarma is a specialized branch of Ayurveda, which deals with both aspects of Ayurvedic treatment which are purification as well as pacification of vitiated Doshas. Virechana (Purgation) is considered as Agra Chikitsa (main treatment) in all types of Kushtarogas (skin diseases) and also given Pradhanyata in Pittaja type of Kushtas. Vicharchika is a type of Kshudra kushta with Pitta and Kapha dosha pradhanyata and has the symptoms like Kandu (itching), Srava (discharge), Pidaka (eruption) and Shyavavarna twacha (blackish discoloration of skin). Vicharchika is often correlated with Eczema based on its clinical presentation. Acharya Kashyapa mentioned it as Sadhya vyadhis (curable disease). The main line of treatment of Vicharchika is Shodhana (purification) and Shamana (pacification). Among the Shodhana karma, Virechana is chiefly advised along with Shamanaushadi. We are presenting a case of Vicharchika, who made a remarkable improvement in the condition with these lines of treatment.

Thyroid gland is the most important gland of the endocrine system. Thyroid hormones manage the metabolic activity of the body. When the secretion of these hormones gets hampered, the thyroid gland fails to complete the metabolic need of the body. This condition is called as hypothyroidism. This problem is a great challenge for both developing as well as developed countries. It is seen 7 to 8 times more common in females than males. The only treatment available is levo-thyroxine tablets which patients have to take lifelong. Due to high prevalence of the disease in the society, the present study was taken to evaluate the efficacy of the management by Ayurvedic principles. In Ayurvedic literature there is no direct reference of this disease. It may be considered due to the disfunctioning of Jatharagni which may leads to the manifestation of this disease. According to the symptoms of this disease, dominance of Dosha may be understand as Pittaand Kapha. Virechanais the best treatment for the Pitta Dusthi and also an effective treatment for the Kapha Dosha. Hence, in present study, total 20 patients of hypothyroidism with TSH more than 5.0 mIU/ml irrespective of the level of serum T3 and T4 were selected and treated with Virechana karma with Araghvadhadi yoga. Effect of Virechana karma was seen extremely significant on symptoms like fatigue, dry and coarse skin, unexplained weight gain, muscle ache, puffiness of face and breathlessness. After the treatment, the average TSH value reduced from 153.03 to 138.41. This is observed that the TSH level was reduced up to 9.88%.

A healthy and clear facial impression plays the important part for the development of the personality. Since ages, Acne vulgaris has always been one of the most burning problem of the adolescents. It affects the most prominent part of our body, the face. Among the multifactorial etiology of acne hormonal imbalance and infections are most important. Acne is a cutaneous pleomorphic disorder of the pilosebaceous unit involving abnormalities in sebum production and is characterized by both inflammatory (papules, pustules and nodules) and non-inflammatory (comedones, open and closed) lesions. Propionibacterium acnes is responsible for the development of acne vulgaris. Acne Vulgaris can be correlated with Mukhadushika or Yuvanpidika on the basis of similarity in the etiopathogenesis, symptoms, treatment and complication. In Ayurveda it is the Shalmali thorn like eruptions on the face of adolescent due to vitiation of Kapha, Vata and Shonita. Allopathic treatment includes treatment for all forms of acne as topical retinoids and benzoyl peroxide, oral antibiotics, steroids etc. But these causes serious adverse effects. So it is necessary to find out effective and safe treatment for Acne Vulgaris. Ayurveda is for it which plays important role through its principles i.e. Shodhana, Shamana and Nidanparivarjana. As described by various Acharya the Shodhanachikitsa includes Vamana, Virechana and Shiravedha. Among Shodhana, Virechana process is the safe and effective treatment, providing long lasting result for better life of patient.

Psoriasis is one among the wide-spreading, chronic, frequently recurring diseases of the skin. Its worldprevalence varies between 0.09% and 11.4% in 2013 which reported in 2006 as in between 0.5% and 4.6%.In Ayurveda, the description of Kushta-roga viz. Vata-Kaphaja kushta finds similarity with psoriasis. Thecommon sites of occurrence of psoriasis are scalp, elbow, knee, earlobes and sole which are alsoconsidered as the location of Vata and Kapha.The science of Ayurveda has anecdotal accounts of success in the management of psoriasis in theactual clinical practice even though it lacks the evidence-based medical dermatology research data.It observed that the physicians adopt several treatment protocols for the cure of this disease inclinical practice, which was not rigorously evaluating in research settings. This paper/report is oneof on the ‘Thuvaraka rasayana’ is one such component that could improve treatment outcomes inPsoriasis.A pre and post test case report selected of a 36-year-old lady patient who diagnosed as having stablepsoriasis vulgaris for last seven years. She was administered with internal and external therapies alongwith Shodhana therapies (bio-cleansing procedures) and then followed by intake of Thuvaraka rasayana.The total duration of the treatment was 43 days, and the Study subject assessed before treatment, aftertreatment and on follow-up for improvement using PASI scoring, and histo-pathological study. All thesymptoms observed in the beginning were found considerably reduced, and the severity also found mild.On the follow-up, it concluded that the lesions disappeared completely and the skin set back to itsnormal texture. Most of the people have become used to spicy fast food day by day. This has taken human being far away from the nature. Nowadays alcohol consumption is also increasing day by day. Ayurvedic texts has mentioned hepatocellular jaundice as Kamala. Ancient Acharyas has also mentioned 搆amali tu virechanama� as Chikitsa Sutra for Kamala. Patient was treated with an integrated approach of Ayurvedic treatment including Virechana Karma (purgation) and Shaman Chikitsa. Due to frequently intake of Pittakara Aahara (spicy and hot food), it leads to vitiation of Pitta Dosha and Virechana (purgation) is the first line of treatment for Pittadushti. Due to Ushna, Laghu, Rruksha Guna, and having Tikta -Katu Rasa of Patola, Amrita, Kutaki (ingredients of internal medication) enhances digestive capacity by hepato-protective action and Pittasaraka guna. Ayurvedic line of management i.e. Virechana therapy and Shamana Chikitsa are successful in Kamala by removing toxic waste from the body, and by correction of Agni (digestive fire). In our ancient classics, single drug along with compound drug have been mentioned in Kamala. These drugs have Kamalahara properties. These Ayurvedic formulations relieve the symptom of Daurbalya (weakness), Kshudha mandya (Appetite loss), Pitamutrata (yellow discolouration of urine), Hrullasa (nausea) and Udarshoola. Also these drugs reduce bile in blood circulation and normalize the other blood parameter. Here a case report of a 50 Years male was having Kamala (hepatocellular jaundice) who was treated with some Ayurvedic herbs and Virechana like Panchakarma and get effectively result with Ayurvedic management.

Amlapitta is one of the leading problems of new era due to irregular and improper diet, dietetics and stressful life style. It is a very common disorder of Annavahasrotasa caused by Vidagdha Pitta with features like, Abipaka (indigestion), Klama (fatigue), Utklesha (nausea), Tikta-Amlodgara (sour or bitter eructation), Gourava (heaviness), Hritkanthadaha (heart burn), Aruchi (anorexia) etc., Amlapitta include acid peptic disorders (APD)- gastroesophageal reflux disease (GERD), Peptic ulcer diseases (PUD), dyspepsia and functional disorder of GI tract, In the present study, aimed to compare the efficacy of Jeerakadya Ghrita Arohanakrama Snehapana (increasing order) followed by Virechana with Trivrit Churnaadi yoga and Patolashunti Ghrita Arohanakrama Snehapana followed by Virechana with Trivritchurnadi yoga in the management of Adhoga Amlapitta. Material and methods: A minimum of 30 patients suffering from Adhoga Amlapitta were selected in 2 groups, 15 patients in each group. Group A- 15 patients were given Jeerakadya Ghrita Arohanakramasnehapana and Virechana. Group B-15 patients were given Patolashunti Ghrita Arohanakramasnehapana and Virechana. Follow up for 14 days (Patients will be assessed clinically on day 0, 7th day & 14th day). Total study duration is 35days. Virechana is highly significant on all the parameters, except Bhrama at both the phases. Statistical analysis of Group A and Group B are showing same result, but there will be reduction percentage in sign and symptoms like Agnimagda, Trishna, Hrillasa, Bhrama, Vividhavarnayoukta Malapravritti Group A is given better result.

Due to altered life style, lack of physical exercise, unhygienic, mental stress, over eating, nutrition deficiency, skin diseases are commonly observed. Psoriasis is chronic disorder which is commonly encountered in day today’s clinical practices. It is one of the burning issues having social importance. Increased stress and sedentary life style are main predisposing factor which is accounting for wide spared prevalence of this disease. All the skin diseases in Ayurveda have been discussed under the broad heading of Kushta. Which are further divided in Mahakustha and Kshudra Kushta. This paper highlights a case study of Kitibha Kushta (Psoriasis) treated with the Ayurvedic principles in particular Shodhana Chikitsa.

Ardhavbhedaka is a type of shirashoola which is explained by all acharyas. According to sushrutha headache which is present in the right or left side of the head, which is splitting, pricking or churning in nature and appeå in the intervals of either 15 or 10 days without any known cause. Acharya vagbhata mentioned that when it is greatly aggravated it destroys either the vision or heå. Migraine is a primary headache involves nerves and blood vessels of the ain, characterised by recurrent headache that are moderate to severe affects one half of the head. Migraine can be triggered by indigestion, hyperchloridria, mental stress, lack of sleep and poor diet etc.virechana is one among the shodhana modality which is performed to eliminate pittadosha from the body through anal route. Though it is agrya for pitta dosha , it also eliminates pitta associate kapha , pittasthanagata kapha and do vatanulomana. charaka mentioned kayavirechana in the chikitsa sutra of ardhavabhedaka.

Psoriasis is one of the most common dermatologic diseases, affecting up to 1% of the world's population. It is a chronic inflammatory skin disorder clinically characterized by erythematous, sharply demarcated papules and rounded plaques, covered by silvery micaceous scale. It has poorly understood etiology and presence of 50% positive family history. Drugs used now a day are basically immunosuppressive which have severe side effects along with remission of disease as well therefore holistic approach of Ayurveda can be a better approach to such disease. Psoriasis due to its different manifestation in different types can be correlated to different types of Kustha mentioned in Ayurveda. So the case being presented here is a male patient with red demarcated patches with severe itching in armpits, groin and nipple. Material and method: The patient was administered with Virechana with Trivritaavaleha followed by Shamana drugs. Results: After Virechana there was the complete improvement in itching and redness of patches was reduced to some extent but after using Shamana drugs there was moderate reduction in all the symptoms along with mild remission. Conclusion: There is need to develop a management for psoriasis which can give benefit on a long run without any adverse effects so, Ayurvedic system of medicine could be answer to this question.

Infertility is a disease of the reproductive system defined by the failure to achieve a clinical pregnancy after 12 months or more of regular unprotected sexual intercourse. As one of the leading causes of anovulatory infertility, it is believed that 5-10% of the reproductive-aged female population is living with polycystic ovary syndrome. Ayurveda considers the excellence of 4 factors- Ritu (ovulatory phase), Kshetra (garbhasaya), Ambu (proper nourishment to embryo), and Bija (healthy sperm and ovum) for a successful pregnancy. Impairment to any of these factors leads to Vandhytva or pregnancy failure. This case study helps to plan a treatment protocol for the patient with PCOS having infertility. A 25 yr old female having regular cycles came to OPD of Streeroga of IPGT & RA, having the complaints of weight gain and failure to conceive since 2 year of active married life. On presentation she was a medium sized woman with android body habits and had mild hirsute and acanthosis nigricans over nape of neck. Gynaecological examination revealed a normal sized uterus with no other abnormalities. Sonography revealed bulky ovaries with multiple small follicles with no evidence of ovulation along with small fibroids on anterior wall (1.8cm×1.5cm) and posterior wall (2.7cm×2.8cm). Her husband’s semen analysis was normal. Based on clinical findings and investigation, anovulatory factor infertility due to PCOS was diagnosed along with fibroid. Virechana and Samana were decided due to both of these factors and Sthanyasodhana gana kashaya was selected as Samana drug. Treatment was done for 3 months, during treatment itself ovulation occured and the patient conceived after 3 months. This case being a Krichrasadhya vyadhi, proper care was taken including correction of the lifestyle and food habits. This case will help to understand the importance of Sodhana in gynecological disorders and explore the probable mode of action of Sthanyasodhana gana kashaya which helped in menstrual regulation.

Virechana (therapeutic purgation) is a common procedure that is widely practiced among the panchakarma treatments (pentad treatments). Various Virechaka dravyas (purgative drugs) have been described for Virechana. Even after critical analysis of Virechaka dravyas in the literature, still there is difficulty in the fixation of dose. Hence, the retrospective analysis of varied outcomes of Virechana with Danti (Baliospermum montanum) avaleha (linctus) is discussed in this paper. The study included twenty‑seven case reports of patients who were administered Virechana with Danti avaleha. These case reports are of patients suffering from various ailments such as irregular menstrual cycles, polycystic ovarian syndrome, primary and secondary infertility, and psoriasis. Danti avaleha was administered at dose of 10 g and 5 g in the Krura (~strong) and Madhyama (~moderate/normal) Koshta (~GI tract) patients, respectively. Among seven Krura koshta patients, three of them resulted with Pravara (excellent) Shuddhi and other four resulted with Madhyama (medium) Shuddhi. In twenty Madhyama koshta patients, sixteen of them resulted with avara (minimum) Shuddhi and remaining four patients resulted with Madhyama shuddhi. Complications like Udara shoola (spasmodic pain of abdomen) and Vamana (emesis) were observed during Virechana. Majority of the patients suffered with Udara shoola were of Madhyama koshta. Vamana was seen in both Krura and Madhyama koshta patients. Irrespective of the type of Shuddhi and complications, all the patients resulted with Samyak Kaphaantiki Virikta lakshana (signs of perfect purgation with end expulsion of Kapha). The study concluded that the Krura koshta patients were tolerable for dose of 10 g and are expected to attain Pravara Shuddhi. Whereas Madhyama koshta patients were intolerable even to mild dose of 5 g, producing Avara shuddhi.
